We are currently accepting applications for a family advocate. It is not likely we will hire anyone until the late Spring/early Summer.

 

Family Advocate- Bilingual (Spanish)

Duties Include:
•Welcome and direct on-site visitors, clients, etc
•Answer and route incoming phone calls
•Develop and maintain community resources and relationships
•Assist in advocating, planning and coordinating activities specific to the needs of the parents, including parent education, workshops and parent support groups.
•In collaboration with the family, provide the following services to the family: needs/strengths assessments, plan development, linkages and consultation, assistance in assessing services, crisis assistance planning, periodic review of needs and progress.
•Perform home visit as needed and conduct appropriate assessments in client’s home.
•Participate at the Family Room at the Truckee Elementary School and aid FRCoT staff and families in preschool activities and run Ages & Stages Assessments as needed.
•Develop and maintain accurate client records, including intake, assessment, service plan, progress notes, encounter logs, and database entry.
•Provide Supervisor with all assigned data and reports in a timely manner.
•Participate and assist in coordination of FRCoT activities and events.
•Attend weekly staff meetings, meeting with supervisor, and trainings as scheduled.
•Maintain professionalism in line with FRCoT policies and standards.
•Maintain a high level of confidentiality as it relates to clients, personnel, the agency and all records/documents.
•Regular attendance.
•Advocate for clients as needed.
•Translate materials for clients and the agency/collaboration.
•Attend community meetings and/or agency/collaboration meetings as assigned.
•Maintains files, logs, charts, database, and other records as required by agency funding.
•Maintains positive communication with all agency staff, clients, and visitors.
•Teach and model parenting skills in clients’ homes.
• Other duties and special projects as assigned.

 

Education and/or Experience:
•Bachelor of Art degree in health or human services related field from accredited college or university, and completion of agency-approved case management training, or;
•Associate degree from accredited college or university, and completion of agency-approved case management training, and two years of experience performing case management duties in a health or human services field, or;
•Completion of agency-approved case management training course with four years experience performing case management duties in a health or human services field.
•Experience in successfully working with infants, children, youth, and their families.
•Community resource awareness.

Language Requirements:
•Bi-lingual English and Spanish conversational and written required.

Salary:
•This is a full time position (35 hours/week) and includes health insurance after 3 month probation period is satisfactorily passed.
